Genetic and Biological Markers in MPNs: How have they influenced clinical practice?

Professors Constantinescu and Skoda started a discussion on the impact of genetic mutations in myeloproliferative neoplasms (MPNs), looking in particular at diagnosis and prognosis. The discoveries of key driver and somatic mutations in the disease area over the past few years has revolutionised the perception on pathogenesis and disease progression. Both speakers highlighted key milestones in the discovery of mutations, with JAK2 V617F being a key player in risk of thrombosis and more recently the CALR mutation and its role in MPNs. Prospective studies in ET in the last 10 years – Where are we now?

Professor Griesshammer looked back over the past 10 years to review key prospective studies carried out in Essential thrombocythaemia (ET) and discussed the main findings with a different perspective in light of recent research. Professor Birgegård supplemented this discussion with latest results of the EXELS study providing his insights on this observational study. Professor Mesa initiated further discussion with both speakers by going into further detail on the results of each of the studies presented and their relevance in clinical practice today. What does the future hold in MPNs?

Professors Kiladjian, Barbui, Vannucchi and Besses briefly summarised where clinical practice in MPNs stands today and hypothesised how it would change in the future. Several approaches were taken with focus given to the role of molecular profiling, evolution of prognostic models and establishing a multidisciplinary collaboration in patient management.
